Belmont Medical Associates participates in most major health plans; however, you should verify your benefits with your insurance company prior to your appointment. If you are uninsured, or if we do not participate with your insurance, one of our financial counselors will be happy to work out a financial plan for you prior to your visit.

Co-pays are due at the time of service and may be paid by cash, check, Visa or Mastercard and Discover. If you do not bring your co-pay, your appointment may be rescheduled.

Our office policy includes a $25.00 no show fee that will be charged to you if your appointment is not cancelled 24 hours prior to your visit.

This practice is open to all patients regardless of source of payment. We will provide uninsured patients information regarding how to obtain insurance.
